Overview

The annual follow-ups are those done at 12 and 24 months after completion of the experimental sessions. On this page, you will find instructions on what to expect when asked to complete your annual follow-up.

What to Expect

The Follow-Up coordinator will contact you by U.S. mail and e-mail when your annual follow-up is due. The annual follow-up consists of two parts:

To complete the online questionnaire, please visit our Annual Follow-Up Survey. The survey is open 24 hours a day.

Once you have completed all five parts, your responses will be submitted to us and we will be contacting you for the phone interview portion.

If you have 15 minutes free, you can always try contacting our lab to complete the phone interview. Our phone number is 773-834-3878 and we are open from 9am to 5pm.

If you finish your annual follow-up before the end of the month, you will receive $40 in gift cards to your choice of Barnes & Noble, Starbucks or Target. If you finish your internet portion by the 15th and we are able to finish your phone interview in a timely manner, you will earn a $20 bonus for a total of $60 in gift cards.

Note: If you have not received a letter or an email notifying you that your follow up is due this month, you will not get credit for completing the survey!
