Overview

The quarterly follow-ups are those done at 3, 6, 9, 15, 18 and 21 months after completion of the experimental sessions. On this page, you will find instructions on what to expect when asked to complete your quarterly follow-up and instructions on how to use our phone system.

If your quarterly follow-up is due this month, you can complete it by calling our toll-free number 24 hours a day! The phone number is:

Note: If you have not received a letter or an email notifying you that your follow up is due this month, you will not get credit for completing the survey!

Quarterly Follow-Up: What to Expect

The Follow-up Coordinator will mail and e-mail you a letter to notify you when your quarterly follow-up is due. To receive your $10 gift card, you need to complete the 5 minute interview before the 15th of the month.

 

You dial the Toll-free Follow-up number 1-866-662-2046, put in your 3-digit subject number, and follow the prompts to continue with the interview.

You will answer several questions about your substance use patterns and moods using our computerized telephone interview (IVR).

The telephone line is open 24-hours, so you may complete the interview at any time of the day.

We will mail you a $10 gift card of your choice (Barnes & Noble, Starbucks or Target) within 2 weeks of you completing the Follow-up Interview (totaling $30/year).

The Follow-up Coordinator will contact you again a few days prior to your 6- and 9-month follow-ups, and the sequence is repeated in the second year of follow-up.

IVR Instructions

Dial the toll free telephone follow-up #: 1-866-662-2046

In order to answer the follow-up questions you will need to first:

  • Enter your 3-digit subject code
  • Enter your birthday using 2-digits for the month and day (ex. "0415" for April 15th)
  • Enter the last 4 digits of your social security number

If you do not enter the correct information, you will be asked to contact Dr. Andrea King at 773-834-2295.

Once you have logged into the system, you will be asked several questions about your substance patterns and moods over the previous 30 days. It may be helpful to have a calendar in front of you. This interview should take no more than 5 minutes of your time. You will be mailed out a $10 gift certificate of your choice (either Barnes & Noble, Target or Starbucks) after you have completed the interview.

Helpful hints to use throughout the interview:

  • Press the # key to repeat the current question
  • Press the * key to back up to the previous question(s)
  • Press 1 for “yes”
  • Press 0 for “no”

The interview includes questions regarding your mood over the past 30 days. You will be using the following scale for these questions:

  • None - 0
  • Slight - 1
  • Mild - 2
  • Moderate - 3
  • Severe - 4

The interview also includes questions regarding alcohol and caffeine consumption over the past 30 days. Use the standard drink equivalence below to answer these questions.

Alcohol standard drinks:

  • 1 standard drink of beer is 12 ounces, or the same as 1 can or bottle
  • 1 standard drink of wine is 5 ounces
  • 1 standard drink of liquor is 1.5 ounces or 1 shot

Caffeine standard drinks:

  • 1 caffeinated drink is a 12 oz soda or 8 oz of coffee or tea
