Click Here to Take the Quarterly Survey Now Overview The quarterly follow-ups are those done at 3, 6, 9, 15, 18 and 21 months after completion of the experimental sessions. On this page, you will find instructions on what to expect when asked to complete your quarterly follow-up and instructions on how to use our phone system. If your quarterly follow-up is due this month, you can complete it by calling our toll-free number 24 hours a day! The phone number is: Toll-Free Follow-Up Line: 866-662-2046 If you are unable to call, you may also complete it online by following the "Quarterly Follow Up" link to the right. Note: If you have not received a letter or an email notifying you that your follow up is due this month, you will not get credit for completing the survey!If you would like to know your subject number or when your next follow up is due, please call our lab at 773-834-3878. Quarterly Follow-Up: What to Expect The Follow-up Coordinator will mail and e-mail you a letter to notify you when your quarterly follow-up is due. To receive your $10 gift card, you need to complete the 5 minute interview before the 15th of the month.

Once you have logged into the system, you will be asked several questions about your substance patterns and moods over the previous 30 days. It may be helpful to have a calendar in front of you. This interview should take no more than 5 minutes of your time. You will be mailed out a $10 gift certificate of your choice (either Barnes & Noble, Target or Starbucks) after you have completed the interview. Helpful hints to use throughout the interview:
